What does the Foundation support?

The Foundation helps create opportunities through training assistance, community basketball programs, equipment support, and academic mentorship. This application is for financial assistance with your athlete’s development.

Do both parents need to upload documents?

For a two parent or guardian household, each adult must provide one recent pay stub or their latest W2 and one recent bank statement. For a single parent household, only that parent’s documents are required. A joint bank statement may be used for both parents if both names are visible.

What if I do not have a pay stub or W2?

Contact our team before submitting. Explain your situation so we can discuss acceptable documentation. Please do not send sensitive financial documents by text or email.

Is assistance guaranteed?

No. Each application is reviewed based on financial need, funding availability, and the athlete’s commitment. Our team will discuss the amount of assistance, your contribution, and any applicable terms with you.

How do we keep assistance active?

Consistent attendance and commitment to the program are required. If an athlete stops attending, support may end. Reapplying may require a new application and placement on the waiting list.

How are our documents kept confidential?

Your application and financial documents are stored privately and used to review your request for assistance. Access through this website requires an authorized Foundation reviewer to sign in. Financial documents are not posted publicly, included in sponsor profiles, or sent as email attachments.

Cover Social Security numbers and full account numbers before uploading. Contact info@leveluphoopsacademy.com with questions about your information or to request removal. Include your application reference, but do not attach financial documents to the email.
